HIT’s Brain Injury Transitional Program provides support for adults with an organic or acquired brain injury. These supports are provided by trained staff at the individual’s home or in the community. Participation in the transitional program is unlimited as far as the duration. Individual circumstances decide what the course length and amount of support needed will be. The mission for the transitional program is to provide structured support where all persons have an opportunity to exercise decision-making capabilities and remain socially active in the community.
Services for participants in the transitional program will be determined by a county case manager but may include the following: Self Care Activities, Housework, Laundry, Meal Preparation, Social, Leisure, and Community Activities including Shopping.
