An Agency Foster Home for Adults. The purpose of an Agency Foster Home for Adults is to provide the setting where four or fewer Medicaid waiver recipients live together and are receiving Residential Habilitation or Community Support services. All referrals for the Agency Foster Home must go through the ADRL and be assigned to an HCBS case manager to determine eligibility for services in this setting. This setting cannot accept private pay.

Day of assessment, individuals meet with the business office for registration, complete behavioral health assessment and Clinical Care Coordinator assists with arranging appointments with behavioral health staff. They are connected for assessment People with acute, severe symptoms are served right away. Others may be served within one to several days or may be referred to other community behavioral health service providers. Clinical Care Coordinator assists with arranging appointments with behavioral health staff. Making an appointment for assessment can save you wait time and ensure you are seen that day. Residential Services: Bridges to Recovery is a 3.5 Substance Use Residential Program that provides clinically managed, high intensity residential services, that are community-based services for individuals with substance use and other co-occurring mental health diagnoses in a monitored supportive environment. The target population for Bridges to Recovery are individuals ages 18+, who present with chronic substance use and possible co-occurring mental health diagnoses. Referrals or inquires for Bridges to Recovery can be sent to bridgesreferral@nd.gov.
